Why focus on senior care?

We believe that the clinical needs of younger adults and older adults are very different. In the same way children are cared for by pediatricians who specialize in child development, we believe that seniors should be cared for in senior-focused clinics by clinicians who specialize in geriatric care. This singular focus allows us to create a healthcare experience for our patients that specifically caters to their needs, such as longer appointments, transportation assistance, community events, lectures, and other services that address not only their health, but also their well-being.

What is calyx health's approach to care?

We understand that seniors’ health needs are complex. Instead of a narrow, symptom-based approach to care, we work with patients and their families to understand their long-term goals and values. Our team takes the time upfront to develop an open, collaborative relationship so we can present a full range of medical options—always with the patient’s goals at the forefront. It's important to us that we're not just treating the body, but the person as well. Our clinic focuses on delivering exceptional care at the primary care level—where we can truly be an advocate for your health—to help minimize unnecessary referrals to specialists.

What can I expect when I come to the office?

You will be welcomed into a modern, refreshing clinic atmosphere and met with a clinical team practicing the latest in geriatric medicine. From a completely redesigned patient check-in process to echo-reducing acoustic ceilings, everything about our office has been designed with seniors in mind. We are on a mission to build the healthcare system seniors deserve, and most importantly, treats seniors with respect.

What services you do you offer?

While we focus on primary care , we are constantly working to expand our clinical offerings. See below for a list of services that are currently available in our office:

  • Annual Wellness Visit & Welcome to Medicare visit: A 60- to 90-minute visit to create a personalized prevention plan. Click here to learn more.

  • Problem-Focused Visit: A standard office visit to diagnose or treat a new problem or follow-up an existing one.

  • Advanced Care Planning: A discussion about “advanced directives” to document a patient’s wishes about future medical treatment.

  • Well Woman Exam: This visit includes cervical or vaginal cancer screenings, pelvic examinations, and breast examinations.

  • Transitional Care Management: An in-person visit following hospitalization or inpatient care to coordinate follow-up care and minimize the chance of readmission.

  • Chronic Care Management: A comprehensive care program dedicated to helping patients manage chronic conditions. It begins with establishing a care plan to manage your health over the long-term and continues with ongoing touchpoints both in the clinic and from the comfort of your home.

  • Behavioral Health Management: A program designed for the coordination and delivery of behavioral health services, such mild to moderate anxiety or depression.

  • Remote Monitoring for Hypertension: A program to monitor a patient’s in-home blood pressure cuff readings using blue tooth technology. In the future, our clinic hopes to deliver connected weight scales, pill bottles, and other connected devices to monitor our patient’s well-being at home.

  • Vaccinations: Administration of the vaccines most relevant for seniors, including the influenza (flu) vaccine, Herpes Zoster (shingles) vaccine, and more.

  • Screenings: Various screening questionnaires used to diagnose alcohol dependency or misuse, depression, and other common conditions in seniors.

where do youR CLINICIANS have hospital privileges?

Our clinicians currently have admitting privileges at Alta Bates Summit and Alameda Hospital. We are in the process of securing privileges at other hospitals in the East Bay. We are also contracted with Affinity IPA, so are able to make referrals to the vast majority of East Bay providers and Stanford Health.

Do you accept my insurance?

We accept Medicare, Medigap/Medicare Supplement, Medi-Cal, and most Medicare Advantage health plans including AARP SecureHorizons/United, Health Net, and Stanford. We do not accept Kaiser.

Is there a fee to join Calyx Health?

We believe in delivering high quality healthcare to all seniors, so there will never be a membership fee to join our practice. If you are on Medicare, we most likely accept your insurance.
