Mabel launched its PY23 HEDR product to help its ACO REACH partners collect demographic data for their 2023 aligned beneficiaries. Within a few short weeks, Mabel was able to help 9 ACO REACHs achieve >90% of their PY23 HEDR adjustment—collectively capturing nearly $2M of incremental profit.

PY23 Learnings & Observations

Mabel’s HEDR product is an administrative solution to collect and aggregate HEDR data across a variety of channels—EMR exports, population health tools, online forms, and paper forms. Reflecting on PY23, ACO REACHs utilized a variety of strategies based on the specific needs of their provider network, thereby maximizing their capture of the HEDR adjustment.

In looking ahead to the PY24 HEDR submission, Mabel has been focused on helping our ACO partners achieve similar or greater success despite the heightened challenges of collecting Social Determinants of Health (SDoH) data.

PY24 SDoH Challenges

  • Most practices do not routinely collect SDoH data

  • CMS only accepts three SDoH screeners (hybrid responses are not allowed)

  • Exporting SDoH data from most EMRs is challenging

Leveraging the insights from our PY23 experience, coupled with novel tools targeting SDoH, Mabel has launched its PY24 HEDR product. In combination with the Mabel Dashboard, which tracks HEDR data collection in real-time across individual providers by channel, we feel confident that our ACO REACH partners will achieve success.
