Best Practices for Obtaining Patient Lists in ACO REACH


Step 1: Prioritize Who

Focus your engagement on the largest providers first. For every ACO, Mabel creates a provider roster on AirTable to help prioritize your team’s engagement efforts and track results. Ask your account manager for your ACO’s AirTable link if you haven’t received it already.

Step 2: Engage Providers

Direct your provider relations team to call provider offices and ask to speak with the office manager or the provider. A sample call script is included below.

Step 3: Follow-up with Instructions

Send a follow-up email with specific instructions based on the provider’s EMR type. Sample emails with instructions for common EMRs are included below.

The fields we request for active Original Medicare / FFS Patients seen in the past two years:

Patient ID (unique identifier assigned by EMR)
Patient Name (can be a single column or broken out as first, last, etc.)
Patient Mailing Address (address lines 1 & 2, city, state, and zip code — either separated as columns or combined)
Patient Email (if available)

Patient DOB
Medicare Beneficiary Identifier (Patient’s 11 digit MBI)
PCP Name
PCP NPI

Step 4: Track Completion

As patient lists stream in, Mabel updates the AirTable roster so you can track progress in real-time / follow-up with providers as needed. Also, continue to refer to the weekly Mabel Status Update for the most recent news and developments.
