ACO REACH: Maximize your PY23 HEDR Adjustment

Under Health Equity Data Reporting (HEDR), CMS is now incentivizing ACOs to collect beneficiary-reported demographic and SDOH data on aligned beneficiaries. As part of this initiative, ACO REACHs can earn up to 0.2% of the ACO Benchmark—representing a significant financial opportunity in 2023 alone.

Mabel’s newly launched HEDR Survey tool helps ACOs maximize their HEDR survey response rates:

  1. Provider-based campaign execution and processing. Similar to voluntary alignment, Mabel sends HEDR Surveys on behalf of providers, and processes/structures the responses that come in across multiple channels (mail, online, in-clinic) so ACOs can easily download a submission file and upload to CMS.

  2. Multi-channel outreach including email. Mabel leverages existing contact information obtained via voluntary alignment activities to outreach patients across email, direct mail, and SMS/text (planned for 2024).

  3. EHR interoperability. Mabel’s data structuring makes it easy to ingest existing demographic and SDOH data from practice EHRs, and export collected data into practice EHR and care management tools, resulting in better economics for the ACO and overall better patient care.

  4. Real-time analytics. The Mabel Dashboard provides real-time tracking of progress including performance by channel and analytics to support provider engagement and future campaigns. As a technology platform, Mabel automatically keeps an audit trail of every outbound and inbound touchpoint, stores records for 10 years, and makes it easy to respond to CMS audits with ease.
